Change a patient's details
Fix a name, update a phone number or add what was left out, right on the chart.
Useful for Secretary Clinic admin Receptionist Doctor Encoder
- Open the patient. Patient Info is the first tab, and its Patient details card holds Identity, Contact, Address, Emergency and Coverage.
- To change a detail, point at it and choose the pencil beside it (on a phone or tablet, tap the detail). Type or choose the new value, then choose the tick to save, or the cross to cancel. Enter saves and Esc cancels. Each detail saves on its own.
- The address is changed as a whole: choose its pencil, pick the Province, the City / municipality (Manila also asks for the District) and the Barangay, type the House no., street, and save.
- To add a detail that is empty, use the Profile incomplete line: choose its name (for example Cellphone or Blood type) and its box opens. The suffix and PWD are there too: choose +N more to see them, under Only if it applies.
- Some details have rules. A cellphone or emergency number is 11 digits starting with 09. The email must be a real address. The date of birth cannot be in the future. Names are kept in capitals. The HMO is chosen from your list, and its Member ID appears once there is one. A Company is picked from the list, or typed to add it.
- The patient's photo is changed with the small button on the picture in the chart header: Upload photo, Use camera or Use webcam, and Remove photo.
- To see afterwards who changed what, and when, open the Audit Trail tab: every change is recorded there.
Common questions
It says "Patient already existing and active" when I change a name.
Another active chart already has that name and date of birth. Search Patients: if it is the same person, carry on in that chart; if one of them is a duplicate, open it and choose Archive patient at the foot of Patient details, and correct the other.
My cellphone number is refused.
It must be exactly 11 digits and start with 09, for example 09171234567. Letters, spaces and a leading +63 are not accepted.
Which details cannot be changed?
The date the patient was registered and the age. The age follows the date of birth. Everything else on Patient details can be changed.
I do not see a pencil.
On a phone or tablet there is none: tap the detail instead. If you still cannot change anything, your role may only view charts (lab staff and encoders).
